Follow the steps below to apply for a Birth Certificate in Assam:
Step 1. The applicant must visit the Public Facilitation Center and the Deputy Commissioner Office in order to obtain a birth certificate application.
Step 2 After filling out the form, you must submit it to the Public Facilitation Office.
Step 3: After submitting the application , the applicant receives a receipt for the application.
Documents required:
- Birth certificate that identifies location of birth.
- Gowanbrae Certificate (Residence proof)
- Proof of parent’s identity.
- Other documents (if needed)
The following person is required to register births in 21 days of the date of birth.
Birth at home: -The household head is entitled to register the birth.
Birth at a public location: -The officer in charge of local police station or the head of the village is entitled to register the birth.
