For shifting residence/Correction or marking of PwD, use Form 8 Voter ID PDF. A registered/enrolled elector can apply for a change of residence or corrections or issue of replacement EPIC.
If the Electoral Registration Officer approves the application for shifting of residence, corrections of entries, and the issue of replacement EPIC with no correction, a replacement EPIC will also be issued to applicant. He must immediately return his old EPIC.

How to file Form 8 Voter ID PDF
- The applicant must mention his name, EPIC number, Aadhaar number and mobile number as well as the email ID of any relative or self. For authentication purposes, an Aadhaar number should be provided. For authentication purposes, applicants who do not have an Aadhaar Number may mention it in box I (b).
- Each applicant must tick the appropriate option to submit a request and complete the required sections of the application. Any other sections that aren't relevant should be omitted.
- The applicant must apply to the Electoral Registration Officer in the constituency where the new address is located.
- The applicant must mention his new address and where he is currently staying. A self-attested copy of any of these documents should be attached as proof of address in his name or the name of his parents/spouse.
- To correct any entry in the electoral roll relating to an applicant, he must tick the box and attach the supporting document.
- The applicant must tick the box requesting a replacement EPIC. The applicant must return the mutilated/old EPIC, or submit a copy FIR/Police report to replace the EPIC that was lost.
